NCLEX Next Generation (NGN) Format 2026: What Nurses Need to Know

The NCLEX changed format in April 2023 with Next Generation NCLEX (NGN), still current in 2026. If you are preparing for the NCLEX as an internationally educated nurse, understanding the NGN format is essential — it tests clinical judgment differently from the old exam.
What Is the NCLEX Next Generation (NGN)?
The NGN is the current version of the NCLEX-RN and NCLEX-PN, launched by the NCSBN in April 2023. It adds new question types designed to measure clinical judgment — the ability to recognize cues, analyze information, and make safe decisions — rather than simple knowledge recall.
How Is NGN Different From the Old NCLEX?
The old NCLEX tested whether you knew facts. The NGN tests whether you can apply those facts to realistic patient scenarios, built around the NCSBN Clinical Judgment Measurement Model (CJMM).

The 6 New NGN Question Types
1. Extended Multiple Response select all correct answers from a longer list, with partial credit scoring
2. Extended Drag and Drop drag responses into the correct order or category
3. Cloze (Drop-Down) complete sentences by selecting from drop-down menus
4. Enhanced Hot Spot (Highlight) highlight relevant words or phrases in a chart or nursing note
5. Matrix/Grid mark responses in a table format (expected vs unexpected findings)
6. Bowtie connect a central condition to actions, parameters to monitor, and likely causes
What Are NGN Case Studies?
Each NGN exam includes 3 case studies. A case study presents an unfolding patient scenario followed by 6 questions stepping through the Clinical Judgment Measurement Model: recognize cues, analyze cues, prioritize hypotheses, generate solutions, take action, and evaluate outcomes. Case studies use partial credit scoring.
How Should Filipino Nurses Prepare for NGN?
• Practice clinical judgment, not memorization focus on why an intervention is chosen
• Use NGN-format practice questions older materials lacking case studies will not prepare you adequately
• Master prioritization frameworks Maslow's hierarchy, ABC, and the nursing process
• Take an NGN-aligned review course NEAC offers online review courses matched to the current NGN format
• Simulate exam conditions practice with the 5-hour time limit and adaptive flow
Does the NGN Change the NCLEX Application Process?
No. The NGN changes only exam content — not application steps. Filipino nurses still choose a US state board, complete CGFNS credential verification, receive ATT, and schedule through Pearson VUE. NEAC manages this entire process.
Frequently Asked Questions
Q: What is the NCLEX Next Generation (NGN)?
A: The NGN is the current NCLEX format, launched April 2023 by the NCSBN. It adds case studies and six new question types that measure clinical judgment alongside traditional knowledge questions.
Q: Is the NGN harder than the old NCLEX for Filipino nurses?
A: Not necessarily harder, but different. The NGN rewards clinical reasoning over memorization. Nurses who practice with NGN-format questions are well prepared. The passing standard is set by the NCSBN.
Q: How many questions are on the NGN NCLEX?
A: The NGN NCLEX-RN ranges from 85 to 150 questions, including 3 case studies of 6 questions each. It is computer-adaptive with a 5-hour time limit.
Q: Do I need new study materials for the NGN?
A: Yes. Materials created before 2023 do not include case studies or the six new item types. Use NGN-aligned practice questions and review courses.
