How to Apply for the NPTE USA Exam Complete Guide for International Physical Therapists
The NPTE is the national licensing exam for physical therapists (PTs) and physical therapist assistants (PTAs) in the United States. International PTs first have their education evaluated (FCCPT charges US$1,240 to US$1,340 and averages 23 to 25 weeks), get approved by a state board, then sit the exam at a Prometric centre in the United States or Canada. The exam costs US$485 to FSBPT plus US$112 to Prometric for PTs.

What Is the NPTE?

- The exam is multiple choice and scored on a 200 to 800 scale. The passing score is 600 at both levels, and every U.S. jurisdiction uses the same standard.
- FSBPT runs the exam four times a year, in January, April, July and October. Registration opens about six months before each date.
- Your score status posts to your FSBPT dashboard about five business days after the exam window ends. A free score report follows ten business days after the exam date.
Who Can Sit the NPTE as an International PT?
FSBPT requires that you are at least 18, have no open registration for the same exam level, and hold a degree from a CAPTE-accredited program or an education that a recognized credentialing agency has found substantially equivalent. Your licensing jurisdiction must also approve you, and it may add its own requirements. Check them before you register.
What Does FCCPT Do, and What Does It Cost?
The Foreign Credentialing Commission on Physical Therapy (FCCPT) evaluates foreign physical therapy education against U.S. standards. FCCPT says its services are recognized by all 53 U.S. jurisdictions. Which service you need depends on the jurisdiction, so confirm with the board you are applying to.

- Processing time starts only after FCCPT has received and verified every document and your payment.
- You need at least a bachelor's degree in physical therapy, and your school must send your academic records directly to FCCPT.
- FCCPT needs verification of every physical therapy license you have held since graduation, active or inactive.
- Documents not in English need the original plus a certified English translation.
- TOEFL scores are required only if the service or your board asks for them. Graduates of programs in Australia, Canada (except Quebec), Ireland, New Zealand, the United Kingdom and the United States are exempt from the English test requirement.
How Do You Apply for the NPTE as an International PT?
- Pick the jurisdiction and read its licensure requirements. FSBPT publishes a licensure reference guide that compares them.
- Complete the FCCPT service your jurisdiction requires.
- Apply to the licensing board. The board approves your eligibility for the exam.
- Register with FSBPT and pay the exam fee. If your jurisdiction has not approved you by the approval deadline, FSBPT closes the registration and refunds the fee minus a $50 processing fee.
- Receive your Authorization to Test (ATT) letter, then schedule with Prometric and pay its fee. Prometric centers fill quickly, so book as soon as the ATT arrives.
- Sit the exam on the date on your ATT letter. FSBPT sends your score to your jurisdiction, which completes your licensure.

FSBPT and Prometric fees add up to about $605 for a PT ($493 with the processing fee, plus $112) and $585 for a PTA, before the FCCPT and board fees. That total is my sum, not a published figure.
Can You Take the NPTE Outside the United States?
FSBPT says the NPTE is offered at selected Prometric centers in all 50 states, Washington D.C., the U.S. Virgin Islands, Puerto Rico, Guam and Canada, about 360 centers in all. It does not list centers in other countries, so international PTs should plan travel to the United States or Canada. You do not have to test in the state where you are applying for a license.
How Many Times Can You Take the NPTE?
- No more than three consecutive attempts in a 12-month period. After three in a row, you must skip the next exam date.
- No more than six attempts in your lifetime at each exam level.
- No further attempts after two scores below 400.
- Your jurisdiction can set stricter limits. After a failed attempt you wait for the next scheduled exam date.
