Furthermore, unless the candidate was educated in an English-speaking country (the United States, Australia, New Zealand, Ireland, the United Kingdom, or Canada – all provinces except Quebec), they must pass TOEFL - iBT or IELTS tests in written and spoken English.

Nonimmigrant healthcare workers who plan to work in one of the above professions must have a valid, unexpired VisaScreen certificate in order to qualify for a temporary work visa, apply for a change of status to work in the United States, extend their stay, leave, return, or apply for an immigrant visa ("green card").
